Application Blank:
Application blank or form is one of the most common methods used to collect information on the various aspect of the applicant’s academic, social, demographics, work related background and references.
It is a brief history sheet of employee’s background, usually containing the following steps:
Ø  Personal Data ( Address, Sex, Telephone)
Ø  Marital Data
Ø  Education Data
Ø  Employment Experience (reason for leaving prev. job)
Ø  Extra- curricular activities
Ø  References and recommendations
